Page 28, between lines 6 and 7, insert:

"Sec. 16. Repeal

Section 47-9521, Arizona Revised Statutes, is repealed.

Sec. 17.  Title 47, chapter 9, article 5, Arizona Revised Statutes, is amended by adding section 47-9521, to read:

47-9521. Uniform form of written financing statement and amendment

a.  a filing office that accepts written records may not refuse to accept a written initial financing statement in the following form and format except for a reason set forth in section 47-9516, subsection b:

B. A FILING OFFICE THAT ACCEPTS WRITTEN RECORDS MAY NOT REFUSE TO ACCEPT A WRITTEN RECORD IN THE FOLLOWING FORM AND FORMAT EXCEPT FOR A REASON SET FORTH IN SECTION 47-9516, SUBSECTION B:
