Strike lines 14 and 15, insert:
"Sec. 4. Delayed repeal
Section 36-761, Arizona Revised Statutes, as added by this act, is repealed from and after December 31, 2018.
Sec. 5. Department of health services; midwives; data; report
On or before October 1, 2017, the department of health services shall report to the committee of reference for the health and human services committee of the senate and the health committee of the house of representatives, or their successor committees, regarding whether ninety per cent of the midwives licensed pursuant to title 36, chapter 6, article 7, Arizona Revised Statutes, have completed the reporting requirements established by the department during the period beginning on July 1, 2014 through September 1, 2017. The committee of reference shall also reevaluate the prohibitions established by section 36-761, Arizona Revised Statutes, as added by this act."
