AMENDMENT EXPLANATION
1. Requires each licensed
health professional to have a telephone number or e-mail address on file with
their health regulatory board to provide to patients who are seeking medical
records.
2. Prohibits a health
professional's residential address and telephone number that is maintained by a
regulatory entity from public disclosure, with certain exceptions.
3. Makes technical and
conforming changes.

entity established pursuant to this title are not available to the
