Overview

Specifies the length of time a pharmacist in charge for a permit issued must provide notice to the board office staff.

History

The Arizona State Board of Pharmacy protects the public and oversees the practice of pharmacy by issuing licenses and permits, investigating and resolving complaints and providing information to the public about the status of licenses and permit holders (A.R.S. § 1904).

Pharmacists, interns, pharmacy technicians or pharmacy technician trainees are required within 10 days after any change of their employer, employer's address, home address or contact information has occurred to electronically update their online board profile or give written notice to the board office staff.

A pharmacist designated as the pharmacist in charge for a permit must give immediate notice to the board office staff of the initiation and termination of such responsibility. The pharmacist must either electronically update the pharmacist's online board profile or give written notice to the board office staff of the new information (A.R.S. § 32-1926).
