3. The number of admissions to screening or evaluation facilities and the places to which members were discharged from these facilities.

4. The number and percentage of members with one arrest and the number and percentage of members with two or more rearrests, the number and percentage of members who were arrested and sentenced to jail or prison and the number and percentage of those sentenced to jail and prison who were released and whether a reentry plan was in place at the time of release.

5. The number of responses by crisis service to members, including the number of members and the locations with one response and two or more responses.

6. The number and percentage of discharges of members from screening agencies because a substance use disorder was deemed to be primary cause of the admission or readmission.

7. The number of member deaths, death rate and cause of death in the preceding year.

8. The number of members who are homeless, unsheltered or inadequately housed and for what period of time.

9. The number of members:

(a) Residing in a behavioral health residential facility.

(b) Discharged from a behavioral health residential facility and the reason for the discharge.

(c) Directly admitted to an inpatient psychiatric hospital within one year after discharge from the behavioral health residential facility.

10. The number and percentage of members who have been diagnosed with a serious mental illness who:

(a) Have received court-ordered treatment.

(b) Have been disenrolled from services that are provided to persons with serious mental illness.

(c) Are compliant with the member's service plan.

(d) Are actively engaged in services.

(e) Are not engaged in services.

11. The number of members employed as a peer support specialist and employed in positions other than as a peer support specialist.

12. The number of patients who have been discharged from the Arizona state hospital and admitted to a contracted psychiatric hospital within the preceding year."
